Our nurses and therapists develop individualized care plans that include each patient’s orthopedic physician’s personally developed protocols for post-surgical care such as knee and hip replacements. We also provide in-home monitoring of anticoagulation therapy that often accompanies orthopedic surgical procedures. This specialized care allows the patient to rehab at home rather than having to enter a rehab facility upon leaving the hospital.
In addition, all patients are given a muti-factor fall risk assessment that reviews such things as: the home environment, current gait and balance, and medical issues and the potential side effects of the patient’s medications that could hinder safe ambulation and the performance of the activities of daily living. Based on the results, education and recommendations are made to enhance the patient’s ability to live independently and safely in their own homes.
