Lexington Health Care has just restructured their Dementia Program Manager for our residents who have early to mid stages of memory loss. We have designed a new program to allow residents to live the most meaningful lives possible within a residential setting, despite their disease process.
In this process we are happy to announce we are currently seeking Dementia Program Manager's who are organized and detail minded individuals to work within our facilities to oversee these programs.
The Dementia Program Manager will serve in a leadership capacity to have day to day oversight of the Memory Care services daily operations.
The Dementia Program Manager will be the process owner to implement and oversee the Memory Care points of distinction established in the areas of philosophy , environment, programming, family services , dining and leadership.
The Dementia Program Manager in Memory Care Services area will complete the assessment and MDS paperwork documentation requirements for residents on an ongoing basis as established within each program by Administrator. Each building may have a different approach to the documentation allocation at the Administrators discretion.
The Dementia Program Manager manages the Resident Service Representative staff and assists the staff in providing the medically related social services and activity programming for the residents in partnership with Clinical Services.
The Dementia Program Manager promotes the facility’s Mission, Visions and Values through role modeling, education and program development for residents, families and staff. The Dementia Program Manager must make them self available on a daily basis to model appropriate quality compassionate interactions with residents living with Memory Loss.
JOB EXPECTATIONS:
The Dementia Program Manager is responsible for the overall management of the Memory Care services area. This includes hiring, directing/supervising, training, competency, counseling, and evaluating of all staff who will work in the memory care area in collaboration with clinical and client services input.
The Dementia Program Manager is responsible for working with customers inquiring about residing within the facility and for completion of necessary documentation that would allow for a smooth transition prior to entering the facility.
The Dementia Program Manager is responsible for developing relationships with residents and families after arrival to the facility and facilitating steps to lead to the highest satisfaction obtainable for each resident.
The Dementia Program Manager is expected to oversee the activity functions on the floor and to assure that all IDPH and federal guidelines are being met for the designated activity program. The MCL is responsible for overseeing compliance with all facility policies and procedures and IDPH/OBRA regulations.
JOB RESPONSIBILITIES:
•Promotion of the facility’s Mission, Vision and Value in all aspects of their role, both for residents, families and staff.
•Implementation and oversight of the Memory Care Services Points of Distinction.
•Daily oversight of the day to day operations in the Memory Care Services area
•Oversee the recruitment, retention, and orientation practices of Resident Service Representative Staff.
•Maintain personnel records of Resident Service Representative.
•Evaluation/performance review of all personnel under the direction of the Client Service Director.
•Management of the Activity program on the designated unit.
•Manages the activity documentation system.
•Organizes the monthly calendar and needed supplies.
•Management of the medically related social services process on the designated unit.
•Works with the staff in the development of social service small groups for the residents who need intervention
•Assists in the process to secure the consent for the resident’s and/or legal representative’s choice for Advance Directives.
•Works collaboratively with the other members of the Client Service division to assure the prevention of customer/resident abuse, neglect and misappropriation of property.
Acts as a resident advocate and communicates with the state Ombudsman as indicated, in conjunction with the Administrator or Asst. Administrator.
•Works collaboratively with the Client Service Director (with Social Service background) for required interventions.
•Coordination of the ancillary services.
•Management and direct implementation of the MDS and Care Plan process as a member of the interdisciplinary care plan team.
•Assist with the census development and marketing of the unit and facility.
•Completes timely and accurate information related to the inquiry, tour and admission processes as well as customer/resident financial aspects.
•Works as a member of the marketing team to meet with customers inquiring about the facility and wanting to admit a resident.
•Conducts marketing contacts in the community (based upon the facility’s marketing plan).
•Assists in the evaluation of the marketing/census development plan and the implementation of recommendations for improvement.
•Other duties as assigned by the Assistant Administrator of Client Services.

| Requirements |

Job Requirements:
•Preference for the Dementia Program Manager to have a 2 year degree in a Health related field or LPN or RN credentials.
•Preferred to have at least 1 year of management experience working with residents with Alzheimer’s disease or related dementias in a long-term care setting.
•Preferred to have at least 1 year of experience of direct oversight of staff
•Possesses excellent communication skills
•A desire and willingness to function in a working supervisor capacity to model interventions and programming with residents living with Memory loss for all staff on a daily basis
The ideal courteous and friendly candidate must have working knowledge of Microsoft Excel and Word along with a minimum of one year of management experience is REQUIRED.
