Lexington Health Network offers integrated care solutions including Home Health Services. Our goal is to help people recover from illness, injury or surgery, avoid unnecessary re-hospitalization and manage long-term conditions with nursing, therapy and other services at home, all coordinated with physicians and other healthcare providers.
Making the decision to have extra help during recovery of a injury or illness as well as long-term support is an important one. We offer a variety of services based on your needs and requirement:
In-home nursing and therapy services when recovering from a major health event, hospitalization, or surgery. We support your recovery and manage your chronic illness, enabling you to rehabilitate in the comfort and privacy of your home. Lexington Home Healthcare offers a diverse menu of services and therapies for Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), Diabetes, general surgery recovery, orthopedics, and wound care. We also support palliative care/symptom management while patients are undergoing chemo therapy. Home Health Care is often covered by managed insurance providers and Medicare.
Lexington Private Care Services provides non-medical personal care and assistance from a compassionate team of caregivers who will help you remain safely and comfortably at home. Private Care and Assistance is privately paid for, but sometimes covered by insurance providers.
We provide comfort and dignity with a holistic perspective, treating the entire patient and supporting their family when illness no longer responds to cure-oriented therapies. Home Hospice and Palliative is often covered by managed insurance providers and Medicare.
