Home Health

In-home nursing and therapy services when recovering from a major health event, hospitalization, or surgery. We support your recovery and manage your chronic illness, enabling you to rehabilitate in the comfort and privacy of your home. Lexington Home Healthcare offers a diverse menu of services and therapies for Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), Diabetes, general surgery recovery, orthopedics, and wound care.

What to Expect

  1. After a consult with our home health team, you will be admitted into a home health plan within 24–48 hours of discharge from the hospital (or at your preferred time).
  2. Therapy services initiate within 24 hours of your first evaluation. A primary nurse case manager and a clinical supervisor is assigned to work with your physician, coordinating care and communication, and facilitating your recovery goals.
  3. You will receive a safety and fall prevention screening upon admission and education to help you manage new medications, diet, and lifestyle changes. Your family is encouraged to participate in the process to aid in your recovery journey.
  4. Services are tailored specifically to your needs and may include the following team members: clinical supervisor, nurse, physical therapist, dietitian, social worker, and home health aides.
  5. Skilled clinical in-home services can include treatments for brain and spinal cord injuries, care for chronic illnesses and injuries, dressing changes, management of tubes, drains, bags, and urinary catheters; medication reconciliation, reports to physicians or pharmacists, orthopedic care, oxygen, tracheostomy care, and wound care.
  6. After close monitoring of progress and overall wellness, you might discharge from home health services or continue services to sustain a healthy quality of life and independence.
