Hospice and Palliative Care

We provide comfort and dignity with a holistic perspective, treating the entire patient and supporting their family when illness no longer responds to cure-oriented therapies. Lexington Hospice partners with you, your family, and your physicians to provide high-quality and compassionate end-of-life care. Lexington Hospice focuses on dignity, symptom control, and emotional and spiritual support for patients in our care. Palliative Care can assist with management of symptoms, without entering a Hospice Program. We also support palliative care/symptom management while patients are undergoing chemo therapy.

What to Expect

  1. Once you and your physician have determined hospice is a possible option, your hospice or palliative team member will meet with you and your family to talk about your unique needs and desires.
  2. After this initial meeting, a team will be assigned to you and your family and will initiate further discussions to ensure your wishes are met.
  3. Continuing care coordinators will assist in your transition home and will set up necessary medical equipment and supplies.
  4. Services may include skilled nursing for comfort and symptom management, nursing assistance for personal care, social work for counseling and emotional support, chaplain guidance, medical equipment and supply management, and therapy services.
  5. We provide ongoing monitoring and updating of services according to the needs and desires of our patients to ensure their comfort and dignity.
  6. Lexington Hospice helps our patients and caregiving families find peace during the end of life transition.
