After an illness, injury or surgery, leaving the hospital is your next step toward recovery. 



Depending on your condition, you may be going home or to another facility for further care. Before you go, it is a good idea to create a list of things you will need. This is called discharge plan. Your health care providers at the hospital will work on this plan with you and your family or friends. This plan can help you get the right care after you leave and prevent a return trip to the hospital.

  1. Schedule an appointment with your primary care physician and make sure that the hospital has sent to your physician all your medical records.
  2. Make certain you have a complete history of all your medications – those you were taking prior to hospitalization, those you took while in the hospital, and those you should be taking after leaving the hospital.
  3. Ensure your home is ready to support your needs.
    Do you have all the supplies you’ll need once you leave the hospital – a walker, wheelchair, recovery bed, oxygen tanks or/ and personal care items?
    Have you installed safety features grip bars for the shower?
    Have you placed things where they’ll be easy to reach?
    Have you stocked the house with foods that meet your dietary requirement?
  4. Make sure you have access to therapy equipment and a rehabilitation therapist.
  5. Confirm that you’ll have friends, family members or caretakers at home to assist you when needed.
